This is a New-Old-Stock joint-army-navy Philips brand 12AT7WA. The WA specifies this to be a special quality version of the 12AT7 tube. The JAN indicates this is a military grade version of the 12AT7. This Philips tube was made in the USA in January of 1988. It features short grey plates and double mica top spacer.
